What are Transition Services?

​

Transition services are supports to assist individuals with disabilities in navigating significant changes in life.  These services help achieve goals related (but not limited) to education, employment, and independent living.  There is no age limit for concierge transition services. 

 

 

 

 

​Daisy Consulting works one on one to create a customized plan based on needs and goals, to support the client and their family.  Service packages to include focus areas that best suit the needs of the individual. 

​

Each potential client will receive a complimentary 30-minute virtual consultation to learn more about the individual, their family, their goals, and the level of support they typically require. Based on this session, Daisy Consulting will create and share options of service plans.​​​
